We are seeking a compassionate, motivated, and highly skilled Physician Assistant (PA) or Nurse Practitioner (NP) to join our dynamic healthcare team. This role is designed for a mobile provider to deliver primary care and wound care services to patients in their homes and Assisted Living Facilities (ALFs) within the Dallas-Fort Worth area. This is a flexible, independent position with the opportunity to work either part-time or full-time, with a focus on geriatric patients who often have complex comorbidities and face challenges leaving their home for medical care.

Geriatric & Wound Care

  • Provide comprehensive primary care services to elderly patients with multiple complex comorbidities, often with limited mobility or safety concerns that prevent them from leaving their homes.
  • Assess, diagnose, and treat a variety of medical conditions commonly found in the geriatric population.
  • Perform wound care services, including debridement and application of advanced skin allograft products.
  • Develop individualized care plans and collaborate with families, caregivers, and other healthcare providers to ensure optimal care.
  • Work independently while managing your daily schedule and patient visits, typically seeing 5-10 patients per day.
  • Maintain accurate and up-to-date patient records, including progress notes, care plans, and follow-up instructions.
  • Ensure a high level of patient satisfaction through personalized care, strong communication, and professionalism.
  • Educate patients and caregivers on health management, including medication management, chronic disease prevention, and healthy aging practices.
  • Drive your own vehicle to visit patients at their homes or ALFs, traveling within the designated area.

Required Qualifications

  • Active Physician Assistant (PA) or Nurse Practitioner (NP) licensure in the state of Texas.
  • Certification as a PA or NP from an accredited program.
  • Previous experience in geriatrics, primary care, or wound care is preferred.
  • Comfortable working independently with minimal supervision.
  • Strong clinical judgment, problem-solving skills, and attention to detail.
  • Experience with wound care, including debridement and advanced skin allograft products, preferred but not required. Willingness to learn and develop these skills is a must.
  • Ability to build rapport with patients and their families, demonstrating empathy and patience.
  • Must have reliable transportation and be willing to travel to patient locations.
  • Must be able to handle the physical demands of the role, including driving and moving between patient homes and ALFs.
